The Metropolitan Center for Mental Health, Inc. (“MCMH”) has terminated the Metropolitan Center for Mental Health, Inc. Defined Benefit Pension Plan effective December 31, 2023. We are planning on distributing or purchasing annuities for all remaining benefits in late 2024. To assist you in returning your completed electronic consent form and completed election forms, we have established this online secure portal in association with Bolton Partners, Inc.; our plan termination service provider calculated the lump sum amounts and optional forms.

Click the button below to upload your completed electronic consent form or completed election package. You will receive an email acknowledgment once the forms have been successfully uploaded.

If you completed an electronic consent form and have emailed and uploaded the signed form, your election form will be emailed to you as detailed in the Notice of Plan Benefits. As a reminder, you must both upload your signed form and email your signed form in order to receive future communications electronically.
